What is Revision Rhinoplasty (Revision Nose Aesthetics)?

Revision rhinoplasty (correction nose aesthetics) is the surgery that has been performed before and to improve the result. Nose surgery may not result in one hundred percent success in the first time. Even in experienced hands, revision (correction) surgery may be required at a rate of 5-15%. These rates are the average of the studies on the results of rhinoplasty performed in our country and in the world. What determines these rates may vary depending on the structure and difficulty of the patient’s first nose, and the surgeon’s experience.

While some of the revision surgeries require minor corrections, some require major operations. In cases where major corrections are required, the operation time can be extended up to 6 hours.

Why is Revision Rhinoplasty (Revision rhinoplasty) surgery needed?

  • The tip of the nose may be drooped, raised, large, compressed, normally too small, or asymmetrical.
  • The nostrils may be significantly different or wide.
  • There may be significant nasal congestion. There may be collapse in the nasal bone or the nasal side walls.
  • There may be a new arch on the nasal dorsum, the nasal dorsum may have collapsed.
  • The curvature in the nasal axis continues or may have re-formed.
  • Healing of the skin and nasal soft tissues may have been problematic.
  • There may be swelling (concha size) from the concha.
  • Adhesion in the nose (synechia) or a hole in the middle of the nose (septum perforation) may have occurred.

When is Revision Rhinoplasty Performed?

It is necessary to wait at least 6 months to decide whether a revision is required after rhinoplasty. The first months are not the right time to decide because the nose will be edematous. The ideal time to plan the revision nose surgery procedure is 1 year.

What is the Difference Between Revision Rhinoplasty Surgery and First Nose Surgery, Why Is It More Difficult?

Revision surgery and primary surgery are different. The texture plans have changed. Some of the cartilage and bones we use in nose shaping were removed in the previous surgery. For this reason, it is necessary to work carefully during the operation. Deformation and incompleteness of normal structures make revision surgeries more difficult than the first. In these operations, the experience of the surgeon and patience during the operation are very important.

Although everything is done properly in revision nose surgery, the condition of the tissues and the harmony of the skin also affect the final result.

In the pre-operative examination, it is important for the patient to have realistic expectations and to understand that there are many factors that affect the result.

What Is Done If The Amount Of Cartilage Is Not Sufficient In Revision Rhinoplasty?

Cartilage is normally used for shaping in nose surgeries. This cartilage is usually found in the septum inside the nose in people who have not had surgery before. In revision surgeries, there may not be enough cartilage in the nose because these cartilages were treated in the previous surgery. In such cases, cartilage grafts from the ear or rib may be required.

Ear Cartilage; It is generally used when minor operations are required. Ear cartilage can be used if minor adjustments are needed on the back of the nose. Since the ear cartilage is curved in structure, it is not suitable for application to every part of the nose. When it is taken with the appropriate technique, there is no shape change or distortion in the ear.

Costal Cartilage (Rib Cartilage): It is suitable for use when a large amount of cartilage is needed in severe disorders or revision rhinoplasty. It prolongs the operation time by about 1 hour. It is very important for the surgeon to be experienced because the study is performed in a region close to the lung. The procedure is performed with an initial incision of 3-4 cm made under the breast on the right side.

Cadaveric Costal Cartilage (Cadaveric Rib Cartilage): Cadaveric cartilage; It is performed by preparing the cartilage taken from the rib of another person by irradiation. Although there is an advantage of not performing additional surgery on the person, melting and bending are more common than the person’s own rib. It is estimated that the procedures performed during irradiation reduce the strength of the cartilage. For this reason, in my own practice, I mostly prefer to take a piece from one’s own rib. 

Synthetic Nasal Implants; Silicon can be counted as Gore-Tex, Medpor and PDS. These are foreign synthetic substances put into the nose. It is not preferred much because of the high complications such as infection and expulsion from the skin.
